NEW PATIENTS
If you're a UK resident and over 16 and would like to register with the Practice please ask at Reception where you will be given a questionnaire to complete with personal and medical details. If you're under 16, you must be registered by a parent or guardian.
These details will be put on the computer and you will need to have a registration medical. You will register with the Practice but you can express a preference as to which doctor you would like to see.
